If you have trouble downloading any of these forms from this page, please send an email to the Webmaster, identifying which forms you would like, and the form will be emailed to you. Or, if this is not practical, please contact the Regional Office and we can send it regular mail.

• Once the form is saved on your computer, you can open it and print it at your convenience.

Player Medical History and Release Form

The use of the USA-Volleyball online registration system for the 2005-2006 season eliminates the need for many of the forms that were previously posted on this page.  The forms will be printed for the registrant during the online registration process.  However, we have found that the Player Medical History and Release Form is quite often not completed at the time, and it can prove to be difficult to locate for later printing.  The form may be downloaded and printed from this link:

Insurance Forms Relating to Member Medical Coverage:

One of the benefits of membership is secondary medical insurance coverage.  If you have been injured in a USAV-related accident, you may use this claim form to recover some of your expenses in excess of your primary coverage.  Please follow the instructions on the form.

Related to this coverage are Incident Reports which should be filed by the coach, official or director responsible for the activity in which the injury was received.  The form has instructions for its use:

Please download all the files and follow the steps found in the Registration Instructions document.  It is also very important this year to use the Data Template form, tested successfully last year, as it will speed up the data entry process in the Regional Office. Please send the Data Template form, on diskette, with the other registration forms to the Puget Sound Office.  If you so desire, the Data Template form can also be sent by email attachment to the Puget Sound Office.  All forms, including the Data Template form, must be completed and signed or they will be returned to you and your registration may be delayed.
